Assessment, education, and skilled care to help the patient return the highest quality of life following a debilitating event, hospital stay, surgery, or chronic medical condition.
Home safety evaluation and to assist in gait, transfer and balance training, prescribed assisted devices (wheelchairs, etc.), increase endurance and functional mobility, physical deconditioning, strengthening exercises, pain management, muscle re-education, therapeutic exercises, and Home Exercise Program
To assist in patient’s independence in the activities of daily living, with increased coordination and strength within a certification period.
Educate and help the patient on techniques to improve difficulties with swallowing, speech, memory training, and communication skills.
Hook up and delivery of medication through an IV.
Delivery of food and/ or fluids directly into the body through an IV.
Assist with activities of daily living like eating, bathing, grooming, and taking vital signs. Work under the supervision of the nurse to make sure the patient’s personalized care is fulfilled.
Evaluate the financial needs of the patient and educate them on community resources, provider services, and counseling needs.
